What is Orforglipron?

Orforglipron, sold as Foundayo, is the newest GLP-1 on the US market — the FDA approved it on April 1, 2026 for adults with obesity, or adults who are overweight with at least one weight-related condition, alongside a reduced-calorie diet and more activity.

What makes it different is chemistry. Every GLP-1 before it was a peptide, which is why they were injected or, in Rybelsus and Wegovy tablets, wrapped in absorption enhancers with strict empty-stomach rules. Orforglipron is a small molecule: it survives digestion on its own, so it can be swallowed at any time of day, with or without food or water.

That also makes it far easier to manufacture at scale than an injectable peptide — the reason analysts expect it to be the first GLP-1 that does not run into supply constraints.

How it works

Orforglipron activates the same GLP-1 receptor as semaglutide, producing the same downstream effects: slower stomach emptying, higher insulin release when blood sugar is elevated, and reduced appetite signalling in the brain.

The difference is structural. As a non-peptide small molecule it is absorbed through the gut without help, has a half-life long enough for once-daily dosing, and is not degraded by food in the stomach — hence the lack of administration rules.

In the ATTAIN phase 3 program, adults with obesity and without diabetes lost an average of 7.5% to 11.2% of body weight over 72 weeks depending on dose. In participants who also had type 2 diabetes, average loss ran 5.1% to 9.6%. That places orforglipron below injectable semaglutide and tirzepatide on raw efficacy — the trade is a pill with no administration rules.
