Does Medicaid Cover GLP-1s for Weight Loss?
Coverage is decided state by state, and it has been shrinking. This is the KFF state list as of January 2026, with the states that dropped coverage and what to ask your plan.
States covering GLP-1s for obesity (January 2026)
KFF counts 13 state Medicaid programs that covered GLP-1s for obesity treatment under fee-for-service as of January 2026:
- Delaware
- Kansas
- Massachusetts
- Michigan
- Minnesota
- Mississippi
- Missouri
- North Carolina
- Rhode Island
- Tennessee
- Utah (funding limited to state fiscal year 2026)
- Virginia
- Wisconsin
Every state not listed was marked "not covered" in the KFF map data. KFF also marks Connecticut, Louisiana, New Hampshire, New Mexico, North Dakota, Pennsylvania and Texas as covering some weight-loss drugs but no GLP-1s.
States that dropped coverage
KFF reports that California, New Hampshire, Pennsylvania and South Carolina eliminated coverage. North Carolina ended coverage in October 2025 during a budget impasse and reinstated it in December 2025, so it is back on the list. The brief adds that a few other states are planning or considering obesity-drug restrictions in state fiscal year 2026 or 2027.
Because this moves quickly, treat the list as a starting point, not an answer for your case.
What to ask your Medicaid plan
- Does the state cover Wegovy or Zepbound for obesity, or only for type 2 diabetes?
- Is prior authorization required, and what BMI or condition criteria apply?
- Does my managed care plan follow the state fee-for-service policy? The KFF count is fee-for-service.
- Where do I appeal a denial?

A federal pilot may change this
KFF describes a federal BALANCE model that lets states opt in for GLP-1 coverage in Medicaid, with rolling implementation from May 1, 2026 to January 1, 2027 and a state application deadline of July 31, 2026. Participation is voluntary and the KFF page does not list participating states, so ask your state agency whether yours joined.
Frequently asked questions
Which states cover GLP-1s for weight loss under Medicaid?
KFF counted 13 state Medicaid programs covering GLP-1s for obesity treatment under fee-for-service as of January 2026: Delaware, Kansas, Massachusetts, Michigan, Minnesota, Mississippi, Missouri, North Carolina, Rhode Island, Tennessee, Utah, Virginia, Wisconsin. Utah is flagged because its funding was limited to state fiscal year 2026.
Is this list current?
It reflects January 2026, and state coverage has been changing. Several states cut coverage over budget pressure, and KFF notes more are considering restrictions in state fiscal years 2026 and 2027. Confirm with your state Medicaid agency before relying on it.
My state is not on the list. Can I still get covered?
Coverage for weight loss is a state choice. A state that does not cover GLP-1s for obesity may still cover them for type 2 diabetes, which is a different approved use. Ask your Medicaid plan which uses and prior authorization rules apply.
What is the difference between Medicaid and the Medicare GLP-1 Bridge?
They are separate programs. Medicaid is run state by state. The Medicare GLP-1 Bridge is a federal arrangement for Medicare enrollees. See our insurance guide for the Medicare terms.
What if my state covers it but I was denied?
Coverage usually comes with prior authorization and clinical criteria set by the state. Ask for the denial reason in writing, then see our denial appeal guide.
